Out Your Patient Information Form
(Patients
are seen by appointment only.)
At
the time of your appointment, you will be asked to provide pertinent
personal information, including
current insurance information. We ask that you bring your
insurance card with you to your appointment.
In addition, we ask that you bring a list of all current medications
that you are taking and the glasses or
contact lenses that you wear. If you wear contact lenses,
please bring a list of the cleaning solutions that you use.
For a complete eye exam, your eyes will be dilated. After
dilation, you will be unable to drive for several
hours. It is important to bring a driver with you to your
appointment.
You can expect your examination to take about 1 hour to 1 1/2 hours.
If special testing is required, the exam
may take longer. We make every effort to minimize waiting
time; however, the time it may take to dilate your eyes cannot be
predicted.
