Payment Options:

Payment for Services:

Payment in full is expected at time of service. Thurmond Eye Associates will be happy to file you Medicare, Medicaid and CHIPS claims for you. We also participate in numerous PPO plans and will file those claims for you. An itemized statement, including diagnosis, will be provided to you at the time of your visit which will enable you to file any other insurance claim (s).

Medicare:
Thurmond Eye Associates is a Medicare Participating Provider. This means that we accept the Medicare
allowable as payment in full for all covered services. Medicare pays 80% of the Medicare allowable after you annual $100.00 deductible is met. You will be responsible for paying you deductible and 20% of the
Medicare. We will file your secondary insurance for you if your secondary carrier accepts electronically
submitted claims from Medicare. If you carrier does not accept electronically submitted claims, you will be given the necessary documentation to file your secondary insurance.

Eye refractions are not covered by Medicare. The refraction is the test that we perform to determine that need for glasses or for a change in your prescription. If you wish to be checked for glasses, you will be responsible for paying the cost of the refraction at the time of your visit.

Medicaid:
In order to qualify for Medicaid benefits, you must present proof of your coverage each month. Please bring your Medicaid card with you to your appointments. Some patients who are covered by Medicaid are eligible for routine eye care. Eligible participants qualify for one routine eye exam every two years for adults and one every year for children.

Private Insurance:
Thurmond Eye Associates is a participating provider in numerous private insurance plans. Please bring your insurance card with you to your visit so that we can verity our participation and your benefits.

For you convenience, optical services are available in each of our office locations. We provide free
adjustments and free nose-pad replacements. We carry a large selection of the latest styles of frames in all price ranges. Most prescriptions can be filled by our in-house lab in one to two days.
